DYNAMICKICK SOCCER INC.

WAIVER AND RELEASE OF LIABILITY**

In consideration of being allowed to participate in any training sessions, programs, leagues, camps, tournaments, private sessions, or other activities organized by Dynamickick Soccer Inc. (“Activity”), I, for myself and/or on behalf of my child or ward, agree as follows:

1. Assumption of Risk

I understand that participation in soccer and related activities involves inherent risks, including but not limited to physical injury, illness, emotional distress, permanent disability, paralysis, and death. I voluntarily assume all risks, known and unknown, associated with participation in the Activity, whether arising from the negligence of Dynamickick Soccer Inc. or otherwise.

2. Release and Waiver of Liability

I hereby release, waive, and forever discharge Dynamickick Soccer Inc., its owners, directors, officers, coaches, staff, volunteers, affiliates, agents, and representatives (“Releasees”) from any and all claims, demands, damages, losses, or causes of action arising out of or related to participation in the Activity, including those caused by the negligence of the Releasees, to the fullest extent permitted by law.

3. Indemnification and Hold Harmless

I agree to indemnify, defend, and hold harmless the Releasees from any and all claims, damages, liabilities, costs, and expenses (including reasonable attorney’s fees) arising from my or my child’s participation in the Activity.

4. Medical Authorization

I certify that I or my child is physically and medically able to participate in soccer activities. In the event of an emergency, I authorize Dynamickick Soccer Inc. to obtain medical treatment for myself or my child, including first aid, CPR, emergency transport, and sharing medical information with medical professionals. I agree to be financially responsible for all medical expenses incurred and understand that I should maintain my own health insurance coverage.

5. Participant Responsibility

I agree that I or my child will follow all rules, instructions, and safety guidelines provided by Dynamickick Soccer Inc. I understand that failure to comply may result in removal from the Activity without refund.

6. Property Damage

I agree to be financially responsible for any damage to facilities, equipment, or property caused by willful misconduct, negligence, or reckless behavior by myself, my child, or any associated party.

7. Media Release

I grant Dynamickick Soccer Inc. permission to use photographs, video recordings, and other media of myself or my child taken during training sessions, events, and activities for promotional, marketing, and educational purposes, without compensation. I understand that I may revoke this consent in writing, and such revocation will apply only to future media use.

8. Continuous Participation Clause

This Waiver and Release of Liability shall remain in effect for all current and future participation in any Dynamickick Soccer Inc. programs, training sessions, leagues, camps, tournaments, or events, until revoked in writing by the undersigned. This waiver applies to all seasons and future enrollments without the need for re-signing.

9. Governing Law and Severability

This agreement shall be governed by the laws of the applicable jurisdiction. If any provision of this agreement is found to be invalid or unenforceable, the remaining provisions shall continue in full force and effect.

10. Acknowledgment and Agreement

I acknowledge that I have read and understand this Waiver and Release of Liability and voluntarily agree to its terms. I understand that by agreeing electronically, I am giving up certain legal rights, including the right to sue Dynamickick Soccer Inc. for injuries or damages.
